Welcome to the Quality Assurance Forms and Applications section. This page provides downloadable forms and application templates required for various academic quality assurance activities at the University of Duhok.
Faculty members, researchers, and academic staff can access the necessary forms to:

Instructions:

- Google Forms can be filled out directly without downloading. - Complete all required fields accurately.
- Attach all necessary supporting documents.
- Submit the completed form to your department's Quality Assurance Coordinator or your QA Officer at your College.
- Google Forms, send a copy of the submission to your department's Quality Assurance Coordinator. - Keep a copy for your records.
